abstract

A number of patients with severe obstruction due to hypertrophic cardiomyopathy have derived benefit at least over the short-term from inventional infarction of a portion of the interventricular septum by the  infusion of alcohol into a selectively catheterized septal artery , with reduction of the outflow gradient and improvement in symptoms . This paper contains success...

Percutaneous transluminal septal myocardial ablation in hypertrophic obstructive cardiomyopathy.

BACKGROUND Percutaneous transluminal septal myocardial ablation (PTSMA) has been introduced as an alternative procedure for reducing the left ventricular outflow tract gradient (LVOTG) in hypertrophic obstructive cardiomyopathy. We report on the acute and mid-term results in 91 symptomatic patients with respect to intraprocedural myocardial contrast echocardiography (MCE). METHODS AND RESULTS...

Five years of percutaneous transluminal septal myocardial ablation.

In June 1994, Ulrich Sigwart injected a small quantity of absolute alcohol into the first septal artery of a 67 year old woman with hypertrophic obstructive cardiomyopathy. The objective of this somewhat alarming manoeuvre was to produce an area of localised myocardial infarction in the left ventricular outflow tract—in eVect producing a chemical myotomy–myectomy, without recourse to open heart...

Percutaneous transluminal septal ablation in hypertrophic obstructive cardiomyopathy.

BACKGROUND Percutaneous transluminal septal myocardial ablation (PTSMA) by alcohol-induced occlusion of septal branches with resulting reduction of LV outflow-tract gradient (LVOTG) is a new treatment option in symptomatic patients with hypertrophic obstructive cardiomyopathy (HOCM). METHODS In 1996 and 1997 we treated 114 symptomatic patients (56 female; age 53.3 +/- 15.6 years; 5 patients w...

Transluminal percutaneous septal myocardial ablation in a patient with hypertrophic obstructive cardiomyopathy.

Reduction of septal mass by inducing septal infarction using catheter techniques is a new therapy for hypertrophic obstructive cardiomyopathy (HOCM). We report a case of severe HOCM that was dramatically improved by this non-surgical treatment. A 60-year-old woman with HOCM had suffered dyspnea (NYHA class III) with syncopal attack despite medical treatment. Left heart catheterization showed a ...
